Single Parent Family Case Manager (Weatherford)

Position Summary:

The Single Parent Family Case Manager provides onsite direct supervision of the Single Parent Family Program. The Case Manager is someone who can enhance program opportunities and services as well as encourage and promote a “can do” attitude among the resident families. This is a full-time, salaried position with benefits. The position does require some evenings, overnight stays and weekends.

Essential Job Functions:

  • Screen and complete assessments with new residential referrals
  • Manage the budget and operating expenses for Single Parent Family Program with Program Director
  • Be an “accountability partner” - assist families in reaching their goals by mentoring and sharing resources
  • Provide case management and outreach services
  • Collaborate with families in developing their plan of service, conduct quarterly evaluations and complete in-home assessments with potential clients
  • Plan and oversee the transition plan for families leaving care and assist with aftercare
  • Be a mentor and problem solver with helping families learn to mediate and resolve internal disagreements and disputes
  • Seek out community resources to enhance programs and services offered in Single Parent Family Program

Job Requirements:

  • Bachelor's degree in social work, counseling or related field
  • 2 years of job-related experience, preferably in case management and residential settings
  • Exceptional organization and time management skills
  • Bilingual a plus
  • Ability to work with both children and adults
  • Proficiency in the operation and maintenance of general office equipment, including Microsoft Word
  • A valid driver’s license and clean driving record
  • Have a reliable and safe means of transportation for self and clients
  • Be able to pass a Criminal History/Background Check
  • Be able to pass random drug testing
